Claims procedure for hospitalisation in network hospitals

If your hospitalisation is covered under your health insurance policy, the HCS or the Health Claim Services or the Insurance Desk or TPA of the insurer, will authorise our “Cashless Service” at any empanelled hospital.

1. Ensure you inform HCS and ensure proper coordination with the hospital. Do not forget to send the relevant details to the HCS to authorise the cashless service.

2. Upon discharge, check and sign the bills of the hospital and pay the due amount that is not covered under your health policy.

3. You should hand over all the reports of tests conducted and other investigations along with the original discharge summary for the claim. Always keep a photocopy of all the documents for your records.

4. If you are aware of the date of getting admitted to the hospital, ensure you coordinate so that they have all the details and can send them to the HCS at least 3 days prior to your admission. This should include the cost estimate as provided to you and the plan of treatment, etc

5. You also need to give the contact details of the hospital to where the authorisation has to be sent.

Claims procedure for hospitalisation in non-network hospitals

To raise a claim in non-network hospitals, you must follow the procedure as outlined below:

1. Inform the HCS at the earliest possible.
2. Settle all hospital bills completely and collect the documents, bills, and reports at the time of discharge.
3. To process your reimbursement, register your claim with the HCS.

To process your claim, ensure the claim settlement form is properly filled out and send it to the insurance company along with the following documentation in either date-wise order or the required format:

1. Your health e-card
2. Your photo ID proof
3. Proof of address
4. Original discharge summary
5. Doctor’s prescription that advises hospitalisation
6. Slip from doctor’s consultation and diagnostic test order.
